The accident reporting register ("Accident Book") should contain the following information:

  • the date and method of reporting
  • the date, time and place of the event
  • personal details of those involved
  • a brief description of the nature of the event.

The Accident Book is to be kept with the AVR first aid kit.

This Emergncy Plan should be reviewed by the club's Welfare officer every 12 months, or if there is a significant change in the clubs circumstances, whechever occurs earlier.
